Due to the entry into force of the Act of December 15, 2017 on insurance distribution (Journal of Laws of 2017 pos. 2486, as amended), (hereinafter referred to as the IDD Act), we hereby address to you the following information in accordance with the requirements of Art. 32 of the IDD Act.

Art. 32 ust. 1 pkt 1 ustawy IDD

As part of the performance of the obligations arising from Art. 32 section 1 point 1 of the IDD Act, we hereby kindly inform you that:

• We carry out insurance brokerage activities under the name: Polish Brokers Group Ltd.,
• our registered office address is: Młynarska 42 floor 4, 01-205 Warsaw,
• we are an insurance broker.

Art. 32 ust. 1 pkt 2 ustawy IDD

As part of the performance of the obligations arising from Art. 32 section 1 point 2 of the IDD Act, we hereby kindly inform you that:

• our entry number in the register of insurance brokers is: 2305/16,
• the register of insurance brokers and the register of reinsurance brokers are available at: https://rpu.knf.gov.pl/search/broker,
• To check your entry in the register of insurance brokers, please visit the website https://rpu.knf.gov.pl/search/broker and search for a broker by the entry number 2305/16 or by the name Polish Brokers Group Ltd.

Art. 32 ust. 1 pkt 6 ustawy IDD

We would like to inform you that we receive a brokerage fee from an insurance company as part of our remuneration fee from the Client with whom an insurance contract is concluded through Polish Brokers Group sp. z o. o.

Art. 32 ust. 1 pkt 8 ustawy IDD

We hereby kindly inform you about:

• possibility of filing a complaint,
• possibility of filing a complaint,
• possibilities of out-of-court resolution of disputes.

Clients of Polish Brokers Group Ltd. have the opportunity to submit a complaint about the company's activities to competent authorities, including the Polish Financial Supervision Authority and the Office of Competition and Consumer Protection.
